Billing Payment Payment for services is the responsibility of the patient or responsible party. Arrangements for payment should be made prior to the initial appointment. Co-pays, co-insurance or other non-covered amounts will be due at each visit. Insurance FVSSC will bill insurance companies with a few exceptions. Coverage of speech pathology services varies widely. FVSSC may be listed as a provider, but your contract may not cover speech pathology services or may have specific limitations. FVSSC will check with the insurance company, but cannot guarantee coverage. Other Requirements All requirements for documentation, such as physician prescription, referral, prior approval, insurance/Medicare cards, Illinois Healthcare & Family Services identification, are the responsibility of the patient or responsible party. Necessary documents must be provided before the patient can be seen. Specific questions about billing can be directed to the Billing Department by Contacting the Center, but please check the FAQ first to make sure your question is not answered there.
