Swallowing Disorders

Learning to swallow or transition to the adult pattern may be delayed. Tongue-thrust patterns cause speech and dental problems. Poor nutrition, dehydration, choking and pneumonia can result .


Referrals are indicated for:


"Tongue-Thrust Pattern"

    when the tongue pushes against the teeth for swallowing, speech, voice and dental problems can occur.

Developmental Delay

    when the transition to the adult swallow pattern has not occured by age 2.

Neurological Involvement

    when brain or nerve function is affected.

Structural

    when the size or shape of oral, facial or pharyngeal (throat) structures affect swallowing. Injury or surgery (such as for cancer) can change structures which are needed for swallowing.


Adults

  • problems may result from stroke, illness, surgery or injury

Children

  • problems may result from premature birth, brain injury or prolonged tube feeding

Services:


Evaluation

  1. Medical history
  2. Oral examination
  3. Observation of swallows of various textures / consistencies
  4. Other testing as needed to rule out complicating conditions

Recommendations

  1. Speech therapy / home program
  2. Medical and/or dental evaluation / intervention
  3. Monitor sleep, breathing, eating patterns
  4. Monitor with nasometry

Therapy

  1. Specific objective goals
  2. Individualized exercises to increase muscle range, force and/or endurance
  3. Exercises to increase sensation
  4. Reduce hyperactive gag response
  5. Reduce / eliminate mouth-breathing
  6. Improve the ability to form and control the food bolus
  7. Establish new muscle patterns

Communication / coordination

    with other professionals and family members.


Diagnosis and treatment may include any or all of the above.
